Fits: Midmark 631 Procedure Chair and Midmark 630 HUMANFORM® Procedure Chair.
Colors: 20 standard and 10 UltraFree®, the same as the 631 and 630 upholstery tops. The SKU suffix is the Midmark color code. Add to your quote with your chair.
